Complex Care Management (CCM) is a set of activities we designed to more effectively assist patients and their caregivers in managing medical conditions and co-occurring psychosocial factors. CCM is usually provided to patients who have serious medical needs and often experience a high number of hospitalizations or emergency room visits. The goal of CCM is to improve the patient’s health status and reduce the need for hospital care.

Our managers work one-on-one with you and your family to ensure that you reach your prime health goals and maintain them.

Complex Care Manager Responsibilities:

• Assesses your needed resources and medical procedures
• Connects with healthcare service providers
• Communicates with healthcare service providers
• Interviews healthcare service providers
• Makes sure your needs are met
• Consults for your feedback
• Follow-ups your progress
